NOTES:

• Multiple Attendees: Submit one form for each attendee.
  
  
• Payment: You will be billed for payment before the scheduled workshop. Reservations in the workshop are guaranteed only with payment in full.
  
  
• Cancellation: Cancellation or rescheduling must be made 14 days prior to the enrolled session to receive a refund, less an administrative fee. There is no refund for cancellations after that point. Substitutions may be made up to 2 days before the start of the session with an administrative fee.
  
  
• Cancellation Policy: Cancellations must be made at least 14 days prior to the program to receive a refund, less a $150 fee. There is no refund for cancellations after that point. Substitutions may be made up to 2 days prior to the program with an additional $150 fee. No substitutions allowed after that point.
  
  
• Reschedule Policy: You may reschedule your registration once to another date. Rescheduling arranged at least 14 days prior to your original program date would incur an additional $150 fee. A reschedule made 13 days or less prior to your original program date will be charged an additional 40% of the original program cost as a fee. Reschedules that are cancelled or rescheduled a second time will forfeit all costs and fees.
